Team Structure: Roster size - minimum of 16 players and maximum of 25 players per team Player Commitment & Attendance Tackle football requires commitment from both players and parents/guardians. Consistent attendance is essential-not just for development, but for player safety. Football is a team sport where all eleven players on the field must understand and execute their roles. Regular practice ensures athletes are prepared to protect themselves and their teammates.
We understand that conflicts can occur and will do our best to accommodate important events such as Back-to-School night. Fundraiser Discount Families can receive a $50 registration discount by participating in our annual fundraiser.

PARENT/ GUARDIAN PERMISSION FOR ATHLETIC PARTICIPATION WARNING:
Although participation in supervised recreational athletics may be one of the least hazardous activities in which any child will engage in, BY ITS NATURE, PARTICIPATION INCLUDES RISK OF INJURY WHICH MAY RANGE IN SEVERITY FROM MINOR TO LONG-TERM CATASTROPHIC, OR EVEN DEATH. Although serious injuries are not common in supervised recreational activities, it is impossible to eliminate this risk.
Participants can help and have the responsibility to reduce the chance or injury. PLAYER MUST OBEY ALL RULES, REPORT ALL PHSYICAL PROBLEMS TO THEIR COACH, FOLLOW A PROPER CONDITIONING PROGRAM, AND INSPECT THEIR OWN EQUIPMENT DAILY.

PARENT/GUARDIAN OR PARTICIPANTS WHO DO NOT WISH TO ACCEPT THE RISK AND RESPONSIBILITIES DESCRIBED IN THIS WARNING SHOULD NOT SIGN THIS FORM OR PARTICIPATE IN THE ACTIVITIES OF THE BLUEDEVIL YOUTH FOOTBALL AND CHEERLEADING CLUB.
LIABILITY WAIVER I, the parent or guardian, or my child, will not hold BLUEDEVIL YOUTH FOOTBALL AND CHEERLEADING program or any officer or coach of this club liable for any expenses that are incurred as a result of my child participating in any activities of the fore mentioned club.
ATHLETIC INSURANCE WAIVER Furthermore, I understand that the BLUEDEVIL YOUTH FOOTBALL AND CHEERLEADING CLUBE DOES NOT provide primary insurance for the children participating in the activities of the club. I understand that I, the parent or guardian, will provide individual insurance for my child or I will assume the responsibility for payment of expenses incurred in the event of injury.
BY SIGNING THIS FORM, I ACKNOWLEDGE AND ACCEPT THE LIABLITY AND ATHLETIC INSURANCE WAVIER AS STATED ABOVE AND THAT I ACCEPT ALL FINANCIAL RESPONSIBILITY SHOULD ANY EXPENSES BE INCURRED. Electronic consent was given during the online registration process.
